Issue with Configurable comment field (22, '102')

When I changed the field name, it does not change on the provider profile when sending out on referrals. You have to select Configurable Comment and that is what shows up on the referral, not the changed name. Without the changed name families have not idea what they are looking at. Is this something that can be fixed? I have attached a screen shot.
